Central nervous system injury caused by a firearm projectile can affect multiple systems, especially when it compromises the somatosensory, visual, and proprioceptive areas, severely altering movement, posture, balance, and sensory-perceptual function. In this case report, the objective is to examine the changes resulting from physiotherapeutic intervention in a patient with severe quadriplegia, motor dysfunction, and multisensory impairment caused by a firearm projectile. The patient is a 31-year-old male with chronic traumatic brain injury due to a gunshot wound in the left frontoparieto-occipital region. He has significant visual impairment, postural alteration, and shortened muscles that prevent him from standing upright. Additionally, the patient exhibits poor eccentric control of the postural muscles, which limits his functional mobility and requires assistance for ambulation. It is concluded that physiotherapy intervention with a multidisciplinary approach in a patient with severe quadriplegia improved motor, sensory, and cognitive function, allowing the patient to regain walking ability and achieve greater autonomy. This progress was also facilitated by the motivation and support of the family.
